This is all wrapped, not sewn. You can do it in MD, but it is very hard to explain. I think Rosemary has some hints for wrapping garments, so maybe she will provide those links.

It looks like a (pleat-less) variation of a dhoti, which I showed here:

Without seeing a better set of photos, it's a bit hard to give specific directions. But, essentially, make a waistband (which can later be made invisible) as a support structure, sew the cloth to that, and then, using pins, pull the cloth around just as it would be in real-life.
